Hormone pellets are small, compressed pellets of hormone, estradiol and/or testosterone for women and testosterone for men, placed under the skin of the upper hip during a brief office visit. They release hormone steadily over a few months. At Peak Medical Wellness, pellets are one option among several. Your provider recommends them when they fit your labs, your history and your routine, and tells you plainly when another method would serve you better.

When your provider may recommend pellets

  • A stable dose has already been established on another method, such as injections or cream
  • Weekly injections or a daily routine have been hard to keep up with
  • For men, no current plans to have children, since pellets, like other testosterone replacement, can suppress sperm production

Pellets aren't the first step for everyone. For women starting testosterone, major medical societies generally favor adjustable methods first, and your provider will explain why.

What to know before choosing pellets

  • The dose can't be adjusted once a pellet is placed. It releases for the full cycle.
  • Women's pellets typically last about 3–4 months and men's about 4–6 months. The timing of your next insertion depends on your labs and how you feel.
  • Testosterone pellets can produce higher-than-intended hormone levels, and some changes, such as hair growth or acne, may not fully reverse.
  • Insertion-site risks are small but real: bruising, infection, or a pellet working its way out.
  • No baths, hot tubs or swimming for 3 days after insertion for women and 7 days for men.

The insertion visit

The procedure is brief and done in the office. After numbing the area, your provider makes a small incision at the upper hip, places the pellets under the skin and covers the site with a small dressing. You can go back to your normal day, keeping the site dry and following the water restriction above. Your provider goes over the aftercare before you leave.

How long do pellets last?

Women's pellets are typically re-inserted every 3–4 months and men's every 4–6 months. The exact timing depends on your labs and how you feel, and your provider sets the schedule.

Can the dose be changed after insertion?

No. Once placed, a pellet releases its dose for the full cycle, which is why your provider usually establishes a stable dose on an adjustable method first.

Are pellets right for everyone?

No. They suit some people's routines well, but for others an adjustable method is the better fit. Your provider recommends based on your labs, history, preferences and, for men, fertility plans.

What's recovery like?

Most people return to normal activity the same day, keeping the site dry and avoiding baths, hot tubs and swimming for a few days as directed. Your provider explains the aftercare in detail.

What happens at the first visit?

You'll meet a licensed provider in person, review your symptoms and health history, and have labs drawn at that visit. Your provider reviews the results with you and builds a plan around them.

Do you accept HSA and FSA cards?

Yes. Peak Medical Wellness accepts HSA and FSA cards. What qualifies depends on your plan and IRS rules, so check with your plan administrator. We can provide itemized receipts.

Treatment eligibility is determined individually based on each patient's health history, current medications, and clinical assessment. We do not prescribe medications online without a prior in-person evaluation.

Pharmacy fulfillment: Medications dispensed or prescribed through Peak Medical Wellness are fulfilled through properly licensed United States pharmacies and compounding pharmacies. Compounding pharmacies are selected based on compliance standards, quality practices, and patient care requirements. All compounded medications are prepared in compliance with applicable federal and state regulations.
