Peak Medical Wellness is committed to delivering medical care that is safe, evidence-based, and grounded in proper clinical oversight. This page outlines how we protect our patients across every aspect of the care relationship — from the providers who direct treatment, to the pharmacies that fulfill prescriptions, to the records that hold your health information.

Provider-Led Care

All medical care at Peak Medical Wellness is directed by licensed advanced practice providers. Patients are seen by Cindy Dean, CNM, APN or Carolyn Bottone-Post, CNM, DNP, FACNM — both certified nurse-midwives and advanced practice nurses with extensive clinical experience.

Treatment plans are not template-driven. Every recommendation is based on the individual patient's history, labs, and goals. Learn more about our providers on the About page.

In-Person Evaluation Required

All prescription therapies at Peak Medical Wellness require an in-person medical evaluation with a licensed provider prior to prescribing. Treatment eligibility is determined individually based on each patient's health history, current medications, and clinical assessment.

We do not prescribe medications online without a prior in-person evaluation. This applies to all hormone therapies, weight-loss medications, peptides, and any other prescribed treatment offered at our clinic.

Prescription Safety & Ongoing Monitoring

Patients receiving prescription therapy are monitored throughout treatment:

  • Baseline labs and intake review medical history, current medications, and contraindications before any prescription is written
  • Personalized dosing rather than one-size-fits-all protocols
  • Scheduled follow-up visits to assess response and adjust treatment
  • Repeat labs as appropriate to confirm safety and efficacy

Patients are encouraged to report any concerns or side effects to the clinic between visits.

Pharmacy Fulfillment & Compounding

Medications dispensed or prescribed through Peak Medical Wellness are fulfilled through properly licensed United States pharmacies and compounding pharmacies. Compounding pharmacies are selected based on compliance standards, quality practices, and patient care requirements. All compounded medications are prepared in compliance with applicable federal and state regulations.

Records, Privacy & HIPAA

Patient health information is collected, stored, and shared in accordance with HIPAA and applicable state law:

  • Patient records are maintained in Optimantra, a HIPAA-compliant electronic medical record system
  • Some records may also be maintained in secure physical formats
  • Patients receive a separate Notice of Privacy Practices (NPP) at intake that governs how medical information is used and disclosed
  • Website inquiries are not HIPAA-secure; patients should not share sensitive medical details through the contact form

Adverse Event Reporting

If you experience a side effect or unexpected reaction to any treatment, contact the clinic as soon as possible:

Peak Medical Wellness reports adverse events to manufacturers and regulatory authorities as required.

Patient Rights

Patients at Peak Medical Wellness have the right to:

  • Receive a clear explanation of any proposed treatment, including alternatives, benefits, and risks
  • Refuse any treatment or discontinue care at any time
  • Request a copy of their medical records
  • Transfer care to another provider
  • File a complaint or concern without fear of impact on their care
